Audit ISO 9001:2015
ermohonan Sertifikasi
Organisasi atau perusahaan yang ingin disertifikasi maka harus melakukan
proses pendaftaran dengan mengisi Form Pendaftaran Klien dan
menandatangani Kontrak Perjanjian Sertifikasi yang berkekuatan hukum dan
disetujui oleh perwakilan kedua belah pihak untuk pelaksanaan kegiatan
sertifikasi. Organisasi atau perusahaan juga dipersyaratkan untuk menyerahkan
(submit) beberapa persyaratan kepada PT. Cipta Mutu Indonesia -
CMIREGISTRARS, seperti :
Legalitas Perusahaan / Organisasi.
Daftar / List Dokumen Prosedur dan Rekaman Wajib.
Rekaman Internal Audit dan Rekaman Tinjauan Manajemen.
Alur Proses Bisnis.
Struktur Organisasi.
2. Tinjauan dokumen dan persyaratan (AUDIT TAHAP 1 )
Tim Audit yang telah ditunjuk sesuai dengan kompetensinya melakukan kegiatan
audit tahap 1 yang meliputi :
A. Evaluasi dan kondisi klien serta kesiapan serta .
B. Mengkaji kesiapan seluruh dokumen yang dipersyaratkan.
C. Mengumpulkan dan mengkaji informasi yang diperlukan untuk
menetapkan program pelaksanaan audit tahap 2 dan evaluasi sumberdaya
pada untuk audit tahap 2.
D. Analisa Rekaman Internal Audit, Rekaman Tinjauan Manajemen, proses
bisnis dan struktur organisasi.
E. Hasil dari kegiatan audit tahap 1 ini akan diinformasikan ke pemohon untuk
ditindaklanjuti.
3. Penialaian Lapangan / AUDIT TAHAP 2 (Main Assesment)
Audit tahap 2 adalah audit komperehensif untuk menilai efektivitas penerapan
atau implementasi sistem manajemen organisasi / perusahaan dengan cara
memverifikasi bukti implementasi secara objektif, mengamati proses kerja dan
wawancara dengan pihak terkait.
Hasil dari kegiatan audit tahap 2 ini akan disampaikan ke organisasi oleh tim
auditor untuk ditindaklanjuti. Tim Auditor merekomendasikan kelulusan /
kelayakan organisasi untuk mendapatkan sertifikat.
4. Verifikasi Tindakan Perbaikan Hasil Temuan
Tim Auditor memverifikasi tindakan perbaikan dari setiap hasil temuan audit,
tindakan perbaikan yang kurang mewakili hasil temuan akan diinformasikan
kepada perwwakilan organisasi klien untuk dilakukan tindakan perbaikan kembali
yang sesuai atau dapat mewakili temuan tersebut.
5. Penerbitan Sertifikat
Sebelum sertifikat diterbitkan, akan dilakukan rapat pengambilan keputusan
sertifikasi berdasarkan dari hasil tindakan perbaikan dan rekomendasi dari Tim
Auditor.
Berdasarkan penerapan yang sesuai persyaratan dan dibuktikan dengan hasil
audit yang memuaskan, PT. Cipta Mutu Indonesia - CMIREGISTRARS akan
menerbitkan Sertifikat
sesuai dengan ruang lingkup penerapan. Sertifikat ISO
yang diterbitkan akan berlaku selama 3 tahun sejak tanggal pengesahannya dan
selama masa berlaku akan dilakukan audit pengawasan (surveillance) minimal
setahun sekali.
6. Audit Pengawasan (Surveillance)
Audit pengawasan dilakukan oleh PT. Cipta Mutu Indonesia - CMIREGISTRARS
untuk memantau tingkat pemeliharaan sistem
manajemen organisasi. Audit Pengawasan dilaksanakan minimal setahun sekali.
7. Audit Pembaruan (Renewal)
Audit Pembaruan akan dilakukan PT. Cipta Mutu Indonesia - CMIREGISTRARS 1 bulan sebelum masa berlaku sertifikasi habis (3
tahun), dengan melakukan audit secara keseluruhan terhadap konsistensi penerapan sistem manajemen organisasi.
